Yes I think you are more or less bang on with this post, it sadly can be the staff sometimes, some just seem to hate the passengers and begrudge giving stuff away. There is certainly no ruling that passengers cant have more juice or toast, and I have never worked a train which has run out of...
However if you try and book the more expensive fare, a message comes up warning you that there is a cheaper fare for your journey, so not that shocking.

OK fair enough 2-3 a week is a lot if your buying them from station ticket offices, the point in having the special stock does seem questionable then.
