As far as the trains I sign go, I'd say the 444s and 450s have decent toilets - usually clean and pleasant enough, functionally on the better side and plenty enough of them (especially on a 444). The only issues are the hand dryers can be temperamental and the CET tanks can make the carriages...
Chain 76 at Hampton Court is a pleasant little place which does good coffee and has rather nice food - independently run and they also offer a loyalty card scheme. Have used it quite a few times when on turnaround there.

I would assume that's fine, providing the bells haven't been given to the driver. At my TOC, we are permitted to re-release the doors for whatever reason at a booked station, though I can't speak for other TOCs.
Certainly true at my TOC, and rather frustratingly when stopped out of course somewhere you're not booked to call, it's often because of a serious incident making it a challenge to contact control because they're so inundated with phone calls!
I'm not too sure - I wouldn't think so as (I've heard) some TOCs don't even permit use of a whistle whilst dispatching, however it's company-specific policy for guards at my TOC.
Having to blow a whistle during dispatch every time, regardless of station or time of day.
In practice, I don't - I see it as a tool to aid dispatch and I see no benefit in doing so at a rural station late at night with nobody on the platform, however it is something I am assessed on.
