Do you have a railcard of your own. If not it will be cheaper to get a family railcard and buy a ticket for your nephew. That way you get a third off your ticket so it works out cheaper and you wont have a problem about anyone asking how old you nephew is.

Just to look at this from the other angle. BR started this process back in the 1980's. On my route for example Reading to Bedwyn is DOO and they closed the ticket office at Hungerford. Now if Hungerford does not have a ticket office why should any of the other similar sized stations ?

No such rule exists. Obviously you have to be careful with minors but I have in the past handed them over to a RPI at a staffed platform.
