A signalling problem, causing the 67 to run through a faulty light onto the back of the MGR hoppers, and wedged under a bridge.

No...it was a braking problem - basically it couldn't stop

I can't remember the exact details but I believe that it was down to staff error rather than a fault with the loco.

333's have not been involved in a collision or fire (unless you count 010's incident with the Cow)

Poor cow. I take it that it hit a cow which was stupid enough to wander onto the tracks? They're heavy beasts, did it cause a lot of damage?

Also, gingerheid, what incident did you have in mind?

The 333 actually hit about ten! How about 321's?

Rulebook definition of an accident:
Taken from module M1 section 1

derailment
collision involving trains or rail vehicles
collision with an obstruction, if the accident might have obstructed other lines
collision with a vehicle at a level crossing
collision with a person
fire on a train which might endanger other trains passing the location
fire on a train which might mean that passengers are evacuated onto running lines
accidental train division

Quite a broad spectrum really :)

how accidental is accidental?

if it's sending a train to the wrong platform that counts, than I think every class is in that ;)

That rules out 444 and 450 then, as Ze Germans successfully drove a 450 into a 444 about a year ago!!

350's out as wll as one had a collission with a tree while on test iirc 350108, the tree was inbeded into the bulkhead behind the driver, after coming through the window.

bet a class 139 has not had an accident yet?

Although it must probably has but has a......357 had a accident.
A quick Google suggests that a couple were damaged in a shunting incident at East Ham depot, with the two good halves being reformed into hybrid unit 357099. This resulted in the loan of, then fellow London Lines TOC, Silverlink's 321408/28 as the timetable required 71 of the 74 units to be in service.

I can't see anything on Weirdy Beardy's site, but Paul Bigland has a shot here.

319s have had their share of de-railments. Last year for example. Under NSE a unit was going round with a driving trailer from another unit due to a derailment. Also one at Bedford where the 319, well, didn't stop in time and went up the buffers.

A 319 also derailed at Luton blocking the North end loop, the slow lines were also blocked by the signaller. The front coach was detached on site and the remaining three coaches were pulled away by a 73. Alledgedly, the driver initially denied he had passed the GPLS at danger and did not mention derailing on the catch point.

Another derailed around the Sutton loop between Wimbledon and Sutton in 2004 after an engineering posession, the trainee driver reporting the part of the rail was missing.

On the subject of incidents and spads, a 319 has made it into the unwired platform 4 at Bedford without aid (quite a feat if you know the geography) and, alledgedly, a driver passed a signal at danger under his own authority (see rule book for details) only to see a train pull out of Luton Airport Parkway station 400 yards ahead of him.

No known pictures of either event.

I believe it was to do with braking (or lack of it), I seem to remember reading somewhere, that the actual cause of the brake failure was due to one of the brake cock being isolated in error (apparently due to the 'instructions' being printed in Spanish (Not sure how true that is, mind!?)) and subsequently the train running away, into the back of an MGR, and I suppose a 'please explain' from the driver's manager! I've seen photos of 67002 in 2003, so it appears it was repaired soon after. It has recently worked for GC, infact.

Also, for crash damaged units, I can't recollect a 323 collision- I'm sure there has been something low profile in the past 16 years though.
