Endorsement means asking the ticket office or authorised staff at the station where the missed connection occurred whether or not they can arrange for a letter to be given to you on headed paper stating you have been given approval to travel on a train that your ticket would not ordinarily...
Should the OP have asked for endorsement on XC? Will the whole delay till the next GWR service be applicable unless the OP asked for the endorsement and was rejected? Just asking as I am not certain
When it's happened to me, once I told guard when boarding and he said he'd come through and do tickets and other time I went to find him ASAP. No issues either time. A TVM that is selectively malfunctioning is a bit more complex to explain. The ones I experienced were showing 'out of order'
My memory is a bit hazy as I'm going back around 25 years and I was only 11 or 12 at the time, but I recall a really long stop on an intercity service possibly Preston or if not one of the other main West Coast stops - for passengers to use station facilities, as I think only one toilet was...
To avoid this perception, I personally have my paper ticket in my hand as I walk past the validators. I do this regularly on long distance rail journeys that end in suburban London at ungated stations

I once had a 10 minute dwell at Plymouth on a Bristol to Penzance early morning class 150 service I boarded at Exeter SD. It was so long the guard announced it and announced the availability of toilets on the platform (better than the train toilet) - most people got off.
