As I have said it does not solve the issue of a train waiting at the last protecting signal south of Leyland waiting for a routing decision, where a platform alteration will always be short notice.
Looking at a track diagram of the area if a Down train is stopped at the last signal protecting the junction from either the Wigan or Warrington direction and the Signaller has to route the train away from its booked platform there is no way the alteration can be posted automatically on CIS...

Presumably because although the ticket was Cheltenham - Sandwell and they were at New Street they had got off a NON PF train, their ticket not being for a journey starting at New Street, I would probably have questioned them under caution and compiled an MG11 in the circumstances.
I accept that PF regs have changed since I last issued one I will have to look up the rules re changing trains. That actually brings up the interesting point that if a PF can be issued when someone is changing trains does can a PF be issued if the train transferred from is a NON PF train...
I don't know if I am missing something here but image of the tickets / itinerary shown earlier does not show a journey starting at New Street the leg being Cheltenham-Sandwell therefore the PF was issued mid journey not prior to a leg starting.
29 used to be Victoria to Enfield and prior to that Victoria to Potters Bar / South Mimms mostly run in sections though there were some through journeys on the whole length of the route. Some Sunday routes in London also used to be quite lengthy combining different weekday routes.
When I was an RPI if we did a name and address check for a PF we would be informed if there had been any other instances of PFs being issued to the name and address, surely NEXUS or their address checking agency could put an alert flag on said address.
If the OP had the Railcard with them when their ticket was inspected then SWT will know when the Railcard expired, if they didn't have the Railcard with them then surely the original post from the OP would have said that they were caught travelling without a Railcard.

Agreed I was trained at King's Cross in the late 70's and we were certainly trained thoroughly in ticketing regulations although to be fair the ticketing system was a lot less complicated then, I last issued an Excess in 2007 at Liverpool Street and we were certainly well trained there then.
