As already stated, the cause of the failure is known, a National Incident notification was issued and fleet-wide check instituted to find any other affected vehicles.

Move along please, nothing to see here...

Guesswork time, from an engineer.

Something either broke or sheared. It did that either because it was worn, fractured, made of the wrong material, included a metallurgical flaw or was to the wrong dimensions.

The first two are routine maintenance issues, the third, fourth and fifth are supplier / manufacturing issues.

That's what requires investigation, long term, as well as an immediate check fleet wide for similar defects.

I assume that the "fleet wide check" also applies to other classes with such couplings, as well as all Mk2 / Mk3 rolling stock.

The cause is known and has been reported elsewhere.

Apparently after a recent examination the buckeye was replaced. The split pin which stops the castellated nut on the shank of the buckeye undoing was not replaced. Over the next couple of days the nut unwound and the buckeye pulled out of its housing. The nut was found on the top of the bogie frame.

Cause known. Nothing to see here, move on...

He is asking 43096 to elucidate to the forum what the known cause of the coupling failure was since it is apparently known to him.

Ah I see. It's one of dem mettyfor fings. :D
