There's a plastic insulation piece between the main body of the interrupter and the rail. I've marked this up on one of your photographs in yellow. There's also (not visible) an insulating ferrule surrounding the bolt that goes through the web of the rail and an insulated washer behind the nut that holds the interrupter onto the rail. I've marked these in green.
Note that it's not a great example of an installation with the use of the L-shaped brackets to terminate the cables. Those brackets should only be used to connect track circuit tail cables to the rail and not onto the interrupter.
The vast majority of interrupters that you'll come across these days will be of the insulated type as that is the safer and modern way of doing things.
