A QR code won't verify the circumstances of a failed claim, most of the time. Only if it was scanned on board a delayed train could that be the case, and in the majority of cases that isn't true.I'd turn it around and suggest that failed claims can be requested to lookup against the ticketing system to verify the circumstances.
Doesn't have to be onboard. If travelling to a barriered station the time of exit will be recorded (usually accompanied by a CCTV image if fraud is suspected).
