I have a query please.

In aviation, there are various means of raising safety related issues anonymously in order that the issues can be addressed and resolved.

I am aware of the O.R.R and C.I.R.A.S and wish to know ( a ) if their are any others and ( b ) which would be the most effective in preventing, the not "if" but "when" event.

The holes in the proverbial Swiss Cheese are now almost fully aligned hence the urgency here.

Any help / suggestions from rail professionals would be most welcome.

If the issue is that serious /urgent, you should inform your Line Manager and his manager and the company CEO by email immediately, if they fail then the ORR.

*When* something happens, the only thing that is clear is that *you* will be to blame for failing in your reponsibilites to report.

You have a responsibility under the health and safety at work act to report any dangerous occurrences as soon as you see them.

If you see something that you consider dangerous enough to describe as urgent then it's YOUR responsibility to stop it from happening.. it may be that the other involved aren't aware of the dangers etc.

I would suggest contacting your line manager ASAP.

Added to the above, I would keep a copy of anything you send to your line manager or anyone else raising this issue.

You will also have a health and safety rep, so you should speak to them. Bigger companies tend to have a 'whistleblowing' policy or procedure too, so maybe that is relevant- my company has a confidential phone line for this kind of thing.

If you feel you cannot report to your line manager. CIRAS should be your port of call.there should also be a dedidcated health and safety manager at your toc who you can discuss things with.

Thank you for the replies so far and my apologies for initially posting on the wrong forum.

To clarify matters therefore.

With regard to the Swiss cheese principle, it's not one single event that gives cause for concern, it's the cumulative series of events that have occurred.

Management have been informed on numerous occasions but simply dismiss these concerns as being irrelevant.

If there was the remotest chance of management taking positive action this would clearly be the obvious course to take.

When management actively, in one instance, condone and encouraged a regulatory breach, then at this point matters require external intervention.

I'm very much an advocate of resolution by means of discussion and safety education. Unfortunately, as I say, there is now no other option other than to raise these issues externally.
