I'm not sure it's something you should be 'looking forward' to reading although the report will be the most reliable way to get all of the facts. Cat A SPAD of this nature is certainly a RAIB matter. For the record, normal working has resumed.

It depends on the precise circumstances; they could well have / will investigate according to their guidelines , but it does not mean to say that they will actually attend in person.

And even a serious SPAD may not be any fault of the driver.

The case on SE last year (?) where the sander-boxws hadn't been filled [ Someone else's job ] & the train slid for 3+ miles ( Stongate ) comes to mind.
Oops, as the saying goes.
