I just included as much as I could. The journey I was doing, the dates and times, details of the the tickets I held (as I was using a combination of tickets), my contact details and address, the length and impact of the delay. Don't think I included anything else.

Include the details of your journey - time it should have started, what trains you got at what time, and how late you eventually arrived at your destination.

Include your address for the voucher.

If you miss out any important details I am sure they will just email you back to ask for them.

True. My main concern was I didn't want to cut the ticket in half (as per their instructions) to find they don't accept email requests and then have to post a defaced ticket.

I wouldn't worry about that (provided it is a Cross country service you were delayed on). If they did want the paper ticket they would be expecting it cut in half.

You could try cutting it using software. That is how the instructional graphic looks to have been done, and you can even see the digital scissors.

I've sent them to XC scanned in without cutting in half, they didn't have a problem with it.

No experience of submitting it online as I wasn't sure I trusted it, and I didn't want to cut my ticket in half. I'd just email everything that is on the form - http://www.crosscountrytrains.co.uk/media/61882/delay_repay_form_sept_2010.pdf
