I've been asked to comment on this thread, specifically to comment on the proposed letter.
I will say that if what is in the letter is true, then there is nothing wrong with it. Such letters rarely receive the amount of attention as this one, nor the, er, benefit of 'design by committee'
Actually, there is an odd sentence in which you say you went to leave the station by a specific exit which you frequently use, and immediately criticise yourself for being mistaken, and apologise, without explaining what was foolish, nor what you are apologising for!
But none of that matters. The letter is one of hundreds that are received that day, and if there's no history on record of past fare evasion by you, then it is unlikely to receive as much attention by Northern as it has on here!
I'd expect a settlement to be proposed.
If there is a history, then the letter is of little value.
As for the 'correction', just paste a replacement page over the one you've superceeded.
In short, this isn't an unusual or complex matter. It appears to me to be an obvious candidate for an administrative settlement.

I received a fixed penalty letter for £86.90 and it's now all been sorted, I am grateful for this result and the help I received here.![]()
