The OP hasn't stated how much the Unpaid Fares Notice was issued for, and East Coast do issue them in respect of excess fares, so it may well be in respect of the appropriate amount for the circumstances. However, to the best of my knowledge, UFN's are not issued by barrier staff at any East Coast station - I am therefore curious to know when and where it was issued.

There are certain station staff at stations with barriers who are trained to carry out UFN. Barrier staff can be trained on UFN if they so wish, subject to a Revenue Protection Course.

East Coast are clamping down on Advance ticket 'get off earlys' as it is mainly used by people who genuinely just want to get off early and their ticket could of been allocated for someone who wanted to get a ticket to that destination but lost out from someone who wanted a cheaper ticket but didn't want to purchase their 'true' destination station.

In this situation yes discretion could of been used BUT then they should of have to pay for a anytime single ticket from Darlington -Newcastle as their ticket was now invalid.

Situations like medical emergency I understand completely and believe discretion should of been given.

The EC advance policy which is being strictly enforced is to make sure that it is fair and equal for people who want cheap advance tickets to their destinations and not for people who purchase tickets and have always decides to board/depart from stations that are before their booked destination station which EC had a lot of and so the revenue protection team needed to step up the game and enforce it.
