If we take it as "Heavy Rail where all services start at Station A, all services terminate at Station B, all services run on the same double track railway (but stopping pattern at intermediate stations can vary)" as a definition then Liverpool Street to Shenfield is the clear winner...

...the Cardiff Bay shuttle takes "silver"...

the "bronze" is tied between Edinburgh - Glagow Queen Street or one of the single Merseyrail services (which are generally all fifteen minute frequencies apart from Ellesmere Port) - unless you want to argue about whether Merseyrail is 100% on Network Rail track, if that is one of the criterion to differentiate Heavy Rail? (due to the long term maintenance of the infrastructure being in PTE hands IIRC?)

(excluding Heathrow Express?)

You said double track. Cardiff Bay is single.

Anyway Stourbridge Town (assuming single track is fine :P) should be joint top, as that's also 6tph.

There are loads of 4tph services beyond the ones you mentioned (mainly in and around London though). Although if you mandate that the calling pattern must also be the same then that knocks a lot of them out, including Edinburgh to Glasgow.

Id have to go for the fast lines through Milton Keynes Central, if somebody ever asked me the busiest lines for night hours between 00:00 and 06:00 then id say Milton Keynes Central. At least something passes through on average every 20 mins during night and 8 minutes in the day

Gatwick Airport probably goes no more than 20 minutes per night with nothing either. I'm not sure why the fasts through Keynes would be specifically the busiest though - afaik most small hour traffic will be on the slows, and during the day the fasts will be just as busy at the likes of Rugby and Watford Jn.
