I have had a few times on TFL where I have claimed against their 15 minute charter and been refunded followed by an automatic refund from the system as well

I had a delay of an hour and a half with Northern, where the last train out of Carlisle to Leeds was 90 minutes late, missing the last train back to Headingley. I put the delay as 60-119 minutes, expecting one free return, but the automated system must have approved it as it reckons I camped out in Leeds station overnight. Because of that, I received two free returns.

LNER refunded me twice for the same journey. I told them about it and was told to keep the money. Got something like £80 plus 2 free first class return tickets. I used them to go from London to Inverness, of course.
