This despite the 6-figure MTIN as recently as 2017? Outdoing all but the very best EMUs as 25-year old DMUs is very impressive.
If you read the Holden report in full you see that that was some interesting accountancy going on to achieve those MTIN figures for example allocating a train fault to staff if it could be shown they had correctly followed processes rather than the unit, at which point unit reliability and MTIN shoots up.
The 6 figure MTIN numbers weren't comparable to other TOCs MTIN data.

It isn’t a unit fault if the staff haven’t operated the unit properly

Why should fleet keep taking a kick in the balls for someone using a worn/broken key or not doing what they have been briefed?

The delay is therefore an operational one and should be flagged to them to get their house in order.

That may well be so and I do think that all TOCs should really be using the same assessment criteria, but I'm not sure I see how, even with such rules, unless pretty much every fault was being discounted, they reached such high figures compared to other DMUs.
