Looking at the NR site, the only services shown for SUY to SHR are those using the through VT services from Euston, so a maximum of three connections including crossing London.

Both Great Eastern and ATW also list services with an extra change at Wolverhampton. Are there two different databases, or are different search engines configured differently?

I notice NR will give me Wick-Barmouth with 4 changes, but in that case there is no 3 change option. Is there a rule that >3 changes is only shown if there is no alternative?

It's not that straightforward, as Kyle-Stranraer* gives a mixture of 3- and 4-change routes. It is certainly doing something, though, as requesting a change in Wolverhampton* causes it to return other options which aren't overtaken by the ones in the original query.

* You may need to delete any nationalrail.co.uk cookies you have in order for the URLs to load correctly.
