Well, there were those three accidents that all happened in 1906, wasn't it; but Salisbury was obviously excessive speed, and according to one interesting theory I saw Grantham may have been because they were in a hurry and forgot to connect up the train brakes to the loco; so Shrewsbury, perhaps?
But then, it was never established why they went through the signal at Harrow & Wealdstone, was it ....
