My partner has been diagnosed with Epilepsy so along with all the documents + PIP, I got her a 3 year Disabled Railcard… only issue is I have rail tickets for us both on 9th & 12th September from Man Vic to York n back with her old 16-25 will she still be able to use them under the Disabled Railcard (As it's the same 1/3 off) or will I have to get them amended at the ticket office? There all advanced 1st class tickets. I'm under 26-30 railcard which is valid until Jan 2025.

You can't get them amended.

Best thing would be to ask the retailer to consider a discretionary, fee-free refund, and then purchase new ones with the correct Railcard - but availability may mean the fare is higher now. Some retailers may support you if you make it clear you will also buy the replacement tickets through them.

May be cheaper just to renew the 16-25 Railcard for 1 year.

I may have misunderstood but if they were purchased with the 16-25 railcard then surely that Railcard must still be valid as you would need to use it with the tickets in September, or is it a case of the 16-25 has expired and instead of renewing that, the Disabled Railcard was purchased instead?

At the time she was about to renew her 16-25 railcard as its ran out on 1st Aug. Today we instead applied for the 3 year Disabled railcard totally forgetting about the tickets we got a few weeks ago that had both her 16-25 and my 26-30 railcards applied. I'll contact the retailer and see what they will say.

Understood, thank you for clarifying.
