The SWR Desiro fleet comes to mind, where there seems to be an ongoing problem with getting them to work - a quick glance at journey alerts shows that no working accessible toilets has become a pretty standard problem across many services daily - anybody know what the issue is?
On the 444s I find the UAT door lock buttons to be rather poorly thought out, with two separate buttons inside both with illuminated rings that really don't make it at all clear whether the door is actually locked or not. IETs suffer with accessible toilet doors which go out of service regularly due to folk trying to pull them open manually and upsetting the sensors, and as somebody mentioned above the 'small' toilet manual sliding doors are getting very stiff to open in some cases. There's also an issue with the placement of the tap sensor, which is quite a way over to the left of the tap itself and causes some users to assume that there is no water available. On both types of train the internal rotating lock handles on the non-accessible toilets can have tendency to turn on their mountings and end up showing locked/unlocked the wrong way around!
Ducking for cover here, but my personal 'favourite' bog would have to be.... Voyagers! I know I know, they stink (but not really all that badly in truth!); but they're a very decent size, easily accessible (speaking as an able bodied person in terms of not having to grapple with iffy doors etc), and the vinyls inside are quite a nice touch. I'll get me coat...
