Doing a bit of rummaging round the net and
this could be the answer ie it's all up in the air at present and noone can make a decision due to union intransigence or other issues. I'm always open to change as long as it's for the best motives - everything in this country at the moment boils down to money and who funds what. That's fine but somebody, somewhere has to have a good oversight and ensure that safety isn't compromised by 'efficiencies'.

I was involved with Flight Safety in a previous incarnation, and am acutely aware of cognitive failures from personal experience. My Flight Engineer inadvertantly switched off the fuel to two donks as we were 5 miles finals to land somewhere out east. Fortunately the other 2 puff jets were fine and the very experienced 1st pilot (not the crew Captain though!) worked through the problem brilliantly and calmly to restart the port engines and land safely without further incident. The co-pilot did a very good impression of a distressed bird, flapping its wings aimlessly! I, meantime, could only pull my seat belt straps tighter(as if that would have helped!)
How we laughed, and changed underwear, when we got off the jet! And how the Eng didn't laugh as he was subject to a full and comprehensive reaming from the Standards Engineer who flew out business class to pump him mercilessly about what happened! Very experienced Eng, but a simple case of Cog failure (ie carrying out a routine action without thinking and getting it wrong) To this day he still has no idea how it happened!
But you learn about stuff from those experiences. What I learned was that even though something is normal, routine, low arousal conditions - you never let your guard down and concentrate on what you're actually doing rather thinking about that first pint after work!
