I would say zero (and this is possibly why there might be confusion) as there is no such this as an 18-25 railcard

There will probably have been some but as it would mean they have paid more than they needed to the likelihood of a TOC taking any kind of action will almost certainly be zero so this forum will never hear about it.

This. It would have to be an incredibly petty inspector who wrote someone up for claiming a smaller discount than they were actually entitled to, and even if they did the chances of a TOC actually doing anything are near zero. So it's unsurprising we don't hear reports of it happening on here.
