The staff who serve the hot food on most LNER services are not chefs, chefs only need to work the evening meal trains and most breakfast trains (if your breakfast has a souffle instead of the fried egg then this is a crew prepared rather than chef prepared breakfast).
East Coast fitted special ovens into their sets to make it easy to deliver the complimentary hot food service. The staff are given a first class product handbook that describes how to prepare the food, there's an example from the veggie chilli:
As for the cups it was not clear where the staff were getting the hot water from. If there was a problem in the kitchen they might not have been able to use their usual jugs of tea and coffee. I've had this issue once when there was an issue with coach H (the kitchen) and it was removed from service. The hot water in first class was provided by an urn on an additional standard class trolley. They used disposable cups in this instance presumably because the trolleys were designed to dispense the right amount of hot water for these cups and they didn't want to overfill the china cups.
I admit it would be crazy if they had their usual tea and coffee jugs if they refused to fill up the regular cups. The attached picture was when it happened to me.
UPDATE: asked the person on Twitter who reported this and he says they were using normal first class jugs for tea, so that bit makes no sense.
