I'd be interested if anyone has an old RailLinks Manual, mentioned by Journey Solutions below, or any lists or examples of these kinds of tickets.

So I asked if they had details. I had these replies from Jonathan Radley at Plusbus ticketing:
"...there is no list of these other National Rail train to bus through fare add-on’s available.
We have suggested to National Rail that they may wish to promote these tickets."
and:
"National Rail used to produce a printed ‘RailLinks Manual’ which listed all the train to bus and tram add-on’s that existed....this manual is no longer produced, but the vast majority of the tickets mentioned in it still exist.
...The individual Train Operating Companies will know what bus and tram add-on’s they manage. So it might be the case of contacting the TOC..."
