What defect would require a second man to be present in the cab?

It doesn't necessarily have to be due to a fault, it could be a manager doing a routine check up on a driver or a new driver having a cab ride to aid their route learning (or even more rarely a signaller with a cab pass seeing their boxes area from ground level as it were).

A competant person may be provided for defective AWS,TPWS,DSD,DRA,Vigilance or a broken/obscured windscreen (ref TW5). This is not exclusive to HSTs.

There is also the case of a driver going over a route which he does not sign and where the 'conductor driver' does not sign the traction.

And example of this many years ago (well, about seven) was a problem on the Midland Mainline and a MML Meridian terminated at Bedford. The train was to go into Bedord Carriage Sidings to wait for it's time to return north. MML drivers didn't sign Bedford Carriage Sidings, so a Thameslink driver was required to conduct the MML driver (who as it happened was a former Thameslink driver) into and out of the sidings.

Some companies also have drivers passing about in lead cans-others outlaw it. I am sure southeastern allow it but southern don't.

Also pilot men can travel in the cans in degraded working.

I was travelling on Sunday from Paignton to Paddington and the train was delayed because of a Cross Country train in front requiring a second driver.
