How did DLR derailment?

you probally heard a couple of months back abouth the DLR derailment,there was no-discovery of how it happened?
or atleast i dont think there was because on the news they also read 'there was no discovery and nothing was found faulty'

so can someone please tell me what caused this accident or
what made it happen,my likely guess is speeding,even though not having a driver.

my suspicion is that there was a fault with the track, as it was at a junction, it could have been damaged or incorrectly set points

Thats on the website above. So i presume that the one driving it from the headquarters must have had inexperience, or just hadn't taken notice properly.

By "manual control" does that refer to the emegancy controls the PSA can use? If so, it won't be speed that was the problem, as they are limited in that mode to about 15mph.

They can go to normal speed under manual control, its where the PSA sits in the front seats and pushes the rubber throttle forwad/back

The only major crash on the DLR happened when i Manualy driven train hit and automatic one. It is likely that the PSA either ignored or didnt notice the signals and the points where set wrong so the train mat have gone over the points at too high speed, casuing it to derail, as the points are sharp.
