The medical is the only part of the recruitment process where you really can get away with wearing casual clothing. What you've described wearing sounds ideal. I rocked up to mine in jeans and trainers, as did a couple of other new recruits I spoke to in the waiting area. The staff at the clinic were fairly casually dressed themselves, as I recall.
I'd still caution against wearing anything overly political or potentially offensive. Leave your nazi uniform/KKK hood and shroud/Orange Order sash/Donald Trump comedy mask etc. at home, and you should be fine.

Haha I was in the same boat a few weeks ago. I'd suggest going smart casual, I'm sure people will have rocked up very casual in the past but it doesn't hurt to make an effort to be tidy. From my experience the Health & Wellbeing Officer and Doctor didn't seem like they were remotely bothered what I was wearing ha.

I'm wondering how some of the more "experienced" drivers deal with that particular problem on their medical renewals, given the average body profile of a good many.

My wife was an HR manager and this question used to come up quite regularly, a good way of looking at it is to consider how you would feel if you turned up looking overly 'casual' and bumped into the person who is to be your line manager, would you feel uncomfortable? a little embarrassed? worried about their opinion of you? or would you not be concerned about any opinion/view they may have?

Very true if going onto work premises. However my TOC medical, in common with most, was conducted at a clinic unconnected to the TOC's business premises.
I'd honestly say, simply wear whatever casual clothing you feel comfortable in, barring anything obscene or offensive. I know for a fact that my manager would have no problem with that.
I'm far more concerned by threads asking whether or not it's appropriate to wear a full suit and tie to an interview or assessment, as sometimes crop up!

A short sleeved shirt which can be unbuttoned really makes the blood pressure and ecg tests quicker and easier!

In all seriousness don't look like you've crawled through a hedge backwards but casual is fine for a medical - they need to be able to get at you easily for things like blood pressure tests.
