Tell me that I live under a rock but I usually don't hear of trains delayed because of passengers as opposed to signal failure.

could be any generic reason, passengers having a medical issue on board and the train needing to wait for paramedics, disorderly passengers, train waiting for BTP, etc etc.

Plenty of times this could happen - issues ranging from fare-evaders (relatively minor) to threats and anti-social behaviour (moderate), to actual physical or verbal agression, vandalism, or a medical emergency.

Its not overly common, but nor is it all rare. Very often on the southeastern network there would be tailbacks for ages in the peak when a passenger was taken ill at London Bridge - a common one in the morning and you find out they've rushed out of the house in winter, eaten nothing (low blood sugar), then got on a busy train with variant temperatures - a fantstic recipe for fainting.

It happens I was travelling on Tuesday (16th) and a northbound CrossCountry service was expected through Birmingham New Street almost an hour late because of 'disruptive passengers' (I looked up later and the trouble was near Oxford). So in a very small sample, it looks almost a daily occurrence somewhere!

"Disruptive passengers" can happen everywhere and anywhere. As an Operations Manager (retired) and a member of public , I have encountered them from the delights of the North London line to the Heart of Wales line.

Of course , in some cases , it is worth having a delay and so on.

It happens I was travelling on Tuesday (16th) and a northbound CrossCountry service was expected through Birmingham New Street almost an hour late because of 'disruptive passengers' (I looked up later and the trouble was near Oxford). So in a very small sample, it looks almost a daily occurrence somewhere!

That was two “disruptive passenger” incidents occurring at the same time at Oxford. The first was a drunk refusing to get off a GWR service that had terminated at Oxford and its next move was ECS to the carriage sidings.
That delayed the following XC service which had already requested police attendance at Oxford due to a disruptive passenger.

I think both trains ended up being platformed at Oxford, permissively, so that BTP could attend both at the same time.
