If a Disabled Railcard holder is travelling from A -> C and is joined by a friend at B, can the friend travel from B -> C on a discounted B - > C ticket? T&Cs say "The accompanying adult must travel with you throughout the journey" but does that simply mean that the friend can't travel without the railcard holder rather than the other way around?

I think this is fine. Especially if station B is unbarriered and the other person can get on the same train as you. I have always read it to mean that the second adult must be with the railcard holder
If you are remaining on the train then how does your companion obtain the discounted ticket?
I recall that a while ago, a small number of TOCs including Northern and (at the time) VTEC had confirmed that was fine with them, but their position may have changed since.
Best to email the train company concerned for an answer and have their approval in writing.
