Recently in an enthusiast group I follow, someone revealed 2x 466s operated the Bromley North shuttle one day last year. IIRC 466s are no longer supposed to be able to operate in passenger service without at least one 465 coupled to them, as this is the condition of the derogation that exists to allow them to continue in service.

I don't know the full details, but it's possible this was a genuine error, and given this branch only takes 5 minutes to traverse, I doubt any passenger with PRM needs was adversely affected (466s have some PRM mods, only wheelchair space and accessible toilets are missing). But this got me thinking, how common are incidences like this, where either the raw legislation, and/or any derogation under it, is breached, whether intentionally or accidentally, around the network?

Defective/incorrect PIS displays seem to be fairly common. Is that permitted?

Would running with all universal access toilets out of order count as a breach? If so then I suspect most operators would be guilty of it.

It is a breach, and needs to be notified to relevant authorities. (ORR think)

However, mitigations can be put in place eg, longer station calls so passengers can utilise station facilities.
