A couple of days ago I, along with a couple of friends, from Caersws to Pwllheli and, to save time, I ordered on-line from TfW.
The ticket details were:
Three tickets from Caersws to Pwllheli. 2 disabled, 1 adults
Price ( correct ) came to £53.80
I attempted to install the tickets into Google Wallet and 10 outward tickets and 2 return appeared.

Looked that the tickets and things got weirder.
I had 3 CWS - PWL but 7 CWS - CWS.
Contacted TfW via WhatsApp and the delightful lady ( Hi Meg) was as puzzled as I was. She checked the booking and, yes, it was correct. She then sent me a link to reload the wallet.
OK ...
Now I had 15 CWS - PWL tickets with 12 being CWS-CWS ( one thing to note that the CWS-CWS were the only ones marked "Disabled") but was up to 3 for the return ..

Anyway I left it as that with 3 apparent valid tickets each way...
All three of us got on the train at 'sws and when the train manager came round I mentioned that I had a real problem with the tickets. He scanned the tickets from CWS-PWL and they seemed to be ok so we went forwards to PWL and had a delightful lunch.
The real fun started on the way back. Showed the three "valid" PWL - CWS tickets and only one was valid. Explained that I had had a problem and showed tje manager (hi Jane!) the WhatsApp chat and the emails confirming the booking.
She then scanned one of the CWS-CWS tickets.
VALID
And another
VALID
In fact ALL of the 12 CWS-CWS tickets were valid!
Journey continued to Mach where we changed trains with the manager joining us.
She wanted details of the tickets so I dug them out again.
She scanned the CWS-CWS tickets again and they were STILL valid .. and again and again ..
We then looked carefully at the e tickets and noticed that the serial numbers of the PWL-CWS tickets were identical as were (in groups of 4) the CWS - CWS tickets!
God alone what went wrong. Jane ( manager) took all the details for investigation because she was baffled, I was baffled and Meg the night before was baffled...
