It strikes me that it's all down to franchise arrangements. Manchester/Lancashire and Yorkshire now share a common franchise which has an effect of integrating the two. So did the West and East Midlands at one time, but this was all undone in 2007. Birmingham - Manchester, on the other hand, has always involved 'crossing a boundary'.
