A piece here from the Hull Daily Mail on the 1927 rail accident near Hull Paragon station in which 12 died and 40 were injured when two trains collided head-on. Telescoping of railway carriages in accidents was a most unfortunate occurrence in the early days of the railways and led to many deaths. Modern carriage construction eventually addressed this problem.

If memory serves, the risk of overriding/telescoping was part of the safety justification for mandating withdrawal of Mk 1 coaches and units. So, more or less 1960s-ish (Mk 2) onwards for such construction in the UK, but after 2000 before nearly all services were using "safe" stock (and Pacers have been demonstrated to have significant issues in that regard). The Mk 1 design did, however, offer much better crash safety than older coaches.
