On the subject of local control, I can't comment on the nature of the failure other than it stopped the job for a while until the crossing was taken under local control, but an AHB near me (linespeed 70mph) failed on Wednesday. Once it was taken under local control, the procedure was something like:
  • Train draws up to signal (auto held at danger)
  • Techs with crossing under local control begin to operate it
  • Driver calls up bobby, who authorises him to pass danger
  • Train crawls over crossing.

The crossing was working with lights on and barriers down and there was only one car so it must have been able to be raised and lowered, but the techs were fiddling in the hut next to it.
Trains were picking up 10-15 mins of delay due to that

--EDIT--
Failed again this afternoon, talked past (but before techs arrived)
