Travelling a couple of days ago on Oyster PAYG, went into Waterloo Underground at the York Road entrance and followed signs for Waterloo and City Line. They took me through two gates and past a validator at the entrance to the W&C, on which I touched.
Looking at my journey history, I entered, exited and entered again and so have been charged a minimum fare for a same station exit. Did I take the wrong route through the station, or should I not have touched the validator?
(Capping meant that it only cost me 20p, but I'd like to know in case there's a next time)

(you were correct to touch in at the W&C validator, otherwise you would have had a maximum cash fare at Bank, which would be in addition to your cap!)
