150/2 separation at Leeds 2018.

Slightly strange Q, but does anyone know much about the Northern 150/2 that separated on the approach to Leeds in early Feb 2018.
I am a trainee driver and we are covering various historic emergency incidents but can find very little on this.
I understand it was quite ‘Political’ as at the time DOO was being discussed, but does anyone have any factual info, the cause etc?

Train uncoupled at Leeds

There are problems for services this afternoon at Leeds, with lots of services currently delayed due to a failure at the western throat. Just leaving on 2H52 15:56 Leeds-Skipton we've passed a 150 (203 I think) that seems to have sat down just outside the station. ETA on NR for normal service...

There was an NIR raised that identified an overly tight jubilee clip holding the uncoupling arm in the coupler slightly away from its correct position.

For reference the NIR number is NIR 3431.
