6A11 (Robeston Sidings - Theale Murco) on fire near Llanelli (27/08)

Thanks,@Cowley for spotting I'd requested the wrong thread to be reopened and sorting it out. The RAIB report shows a woeful lack of oversight of maintenance and failure to learn from previous incidents.

Branding and marketing blunders or failures on the railway.

I remembe seeing one of those posters, underneath "We're Getting There" someone had written in marker pen "(but we're not taking you with us)"

Collision and derailment near Salisbury (Fisherton Tunnel) 31/10/21

What I was getting at was the BBC had changed their story from a stationary train being run into (original claim) to both trains arriving at the junction at the same time and colliding.

Cowden was down to BR cost-cutting. If the line hadn't been singled the accident could never have happened.

Collision and derailment near Salisbury (Fisherton Tunnel) 31/10/21

Re: Detonators Apart from only having 7 mins to cover a decent distance on foot, on rough terrain, this was made twice as difficult because there were two routes to protect in the rear of the train. Put yourself in the guard's position. Assuming not injured and fit enough to do so, which...

Orange line just below train roof

On the Southern Region, stripes at cant rail level used to be different colours for different things. Yellow indicated a first class compartmente, red indicated a buffet car originally, and latterly compartment stock. The former would have been in use from at least the early 1960s.

Lorry backs over LC and breaks barrier

The lorry didn't seem to be going that slowly, even in reverse. "Slow" is defined as 5mph or less. Seems that the lorry was doing much more than that.

Level crossing problem on a heritage railway.

There is a half-barrier both sides, the road being single track. HR train is flagged across. It may be that the crossing is automatic for NR trains and not operated by the HR, hence the need to stop and proceed.

    Accident on NYMR, 21-9-21

To me, this incident sounds like what would have been descibed in olden days as a "heavy shunt".
