The problem is that the cheap crap headphones that come with iPhones and many other devices are inefficient, and spew sound out into the surrounding area, meaning the user turns it up even louder in order to hear it properly.

In addition there is the positive feedback loop that occurs when someone else is listening to music too loudly so someone else raises theirs and so on

I disagree. I have Sennheiser in-ear headphones (which weren't expensive, I think they were around £25) which I can use and are not audible to others. There are similar on-ear styles that work as well. I have to agree with that. My iPod headphones (the ones which come with them) recently broke, so I went down to HMV and got a set of JVC ones for a fiver. They are about 10x more comfortable, quieter to the outside and louder to the user than the Apple ones!

The East Coast quiet coaches in the 225 sets should be moved further down as they arent really quiet with the noisy class 91s infront of them and yes the tables should be banished from the quiet coach

That's one noise I don't mind hearing in the quiet coach personally

With all the local council cutbacks maybe the train companies could employ former librarians to walk up and down the quiet carriages 'shushing' at anyone who makes undue noise.

I think tables in quiet coaches should be kept as sometimes I like to work/read and eat a meal without some little jimmy distracting me with his shouting some trash.
