I have an annual ticket issued by SWT and wish to get a Gold Card Partners Card issued for a friend.

The SWT blurb says that the railcard gets the same discounts as the season ticket holder ie 1/3rd off etc.

Is there still a minimum fare on discount tickets purchased with the Partner Card Monday to Friday or can the ticket be used to buy tickets of any price after 10am on a weekday.

The Partner Card is a standard Network Card with all it's extra restrictions, unfortunately. The only concession is the price of the card.

Do you have to fill in a form to get it? If so, is it the same as the Network card form?

Yes you just fill in the normal network card form. You need to have the Gold Card holder with you, so they can show their valid card. In the past the booking office clerk has written 'Network Card issued' on the back of the Gold Card, although that didn't happen last time mt partner renewed

I have had one of these issued to me a few times and all that has been asked, of the chum whose annual it was, was my surname, which is now printed on said card. That applied when the partner card was issued both at the same time as the season and subsequently.

That is merely my experience; we have booking clerks on the forum clerks who will be able to add their knowledge of the actual rules.

Your normally complete a Network Card Application and are given a receipt for the card so that you can get a duplicate.

The reason that I asked this question is that the person I got the card for has lost their card and applied for a replacement using said receipt.

The first card had Network Gold Card on it and inadvertantly has been travelling Monday to Friday selecting Gold Card Discount and sometimes paying less than the minimum fare. It had a price of £1.00 on it.

The replacement card simply said Network Card and has a price of £28.00. It retains the same expiry date as the original card and it says replacement on there.

On the SWT Season Ticket brochure it does say that the partners Gold Card can be used to get the same discounts as the Gold Card Holder which clearly wasn't the case if a mimimum fare applies.

Clerks are supposed to write this (or similar) on the back so that the annual season ticket holder does not claim two or more partner cards on the same season ticket. I used to write 'GCPNC [Ticket number]' on them, but I forget what the official term is.
