6R03 - Autoballaster derailment at Mitre Bridge Junction during possession

6R03 has derailed across WM2340a points at Mitre Bridge Junction. There is OLE gantry damaged reported. Service suspended Willesden Junction HL - Clapham Junction. S&T have identified damage to WM2340 points and are recovering spares. Source: internal logs

Looks to be an odd alignment for the train, even when derailed - was it reversing/top and tailed?
Looks like it’s derailed over a set of points, front of train gone straight on, rear part attempted to go to the left

3 Auto Ballaster wagons derailed whilst a propelling movement was being undertaken. Points have been ran through and OLE gantry damage.

I see the area around mitre bridge is still under T3. Any idea when things might reopen?
The possession has been shortened to the area around the affected train and line is expected to be handed back sometime tomorrow. All Southern service through the area is suspended until End of Service Monday 22 Jan.

When I was there this afternoon the Overground was running a half hourly shuttle from Clapham Junction to Shepherds Bush, the remaining Stratford to Clapham Junction trains are diverted into Richmond. Expect overcrowding tomorrow - the Overground driver on my train said the line would be out for a few days.

Reports I’ve seen suggest at least one wagon will take 24 hours to lift/remove. Also a specialist team to attend to the leaving OHL structure which a wagon is resting on.
Once the 3 derailed wagons are removed, then obviously infrastructure needs attending to.
I would expect late tues possibly Wednesday before it’s fully open.

No chance. Happened in a possession, during work, no injuries, minor damage. Not the sort of thing RAIB is remotely intersted in.
They are not attending, but want all downloads and investigation documents forwarding to them.

Confirmed as the S&C being run through.
A run through of points does mean that the ES (in a worksite) or PICOP (outside a worksite) were not doing their job properly (making sure all points are set correctly before authorising any movement). And the driver of the train did not check that the points were set in the correct position before proceeding over them.

Looks like points have moved beneath the wagon that is sitting at 90degrees
The same thing happened under a works train at International Junction, Waterloo, in 2017. After the August fiasco of the passenger train being routed into the side of a works train I contacted RAIB about it. The reply was that they were aware of the earlier incident. I do wonder if the same finger trouble was to blame?

Another potential complication in this case is that Mitre Bridge Junction is the boundary point between
Wembley and Three Bridges signalling centres, although I understand that the junction points themselves
are controlled from Wembley.

That was a different issue.
At Mitre Bridge the points were run through in the trailing direction by the loco and front three Autoballaster wagons. When it started moving the other way (facing direction) the part of the train that run through the points went on a different line to the rest of the train. The cause at Waterloo was they were wired incorrectly.

Not necessarily. The points have reportedly been 'run through' ie a trailing movement has taken place with the points set in the wrong lie.

Now set back - the points (now facing) will still be lying as previously set, or possibly not fitting up at all, depending on the type of point operating equipment fitted and the weight of what has committed the run-through.

Powered points that have been run through (in the trailing / converging direction) will cause problems if the train reverses and now moves in the opposite (facing / diverging) direction. Depending on how bad the damage is, axles may go in the position (direction) the points were last commanded (which is not the line it would have come from) or they may derail.

Yes indeed! One I was involved was an engineering train which left a possession without authority and ran through switch diamonds. That was bad enough, but then well-meaning staff on site authorised the train to set back to clear the line, promptly derailing it.
