Just looked on Avantix NFM06 and it seems there are no fares from Three Bridges to Boundary Zone 6, only to Boundary Zone 5. Is this right?

There should be no fares from anywhere to Boundary Zone x.

Boundary Zone x is for the origin only, they are Travelcard extensions and only to be issued in conjunction with a Travelcard.

If Avantix Traveller NFM06 shows fares to any of the Boundary Zones then that must be a mistake. Look up the fare from Three Bridges to Boundary Zone 6, to see how much it costs for a Travelcard holder to make a journey to Three Bridges.

Boundary Zones can be done as a destination, particularly on FGW where staff are told if fare available it should be used rather than doing West Drayton or whichever station applies for zones held.

I assume that the price of the "BZ6 - Destination Station" ticket is the same as the "Last Station in Z6 - Destination Station" ticket. i.e. the BZ6 - High Wycombe ticket costing the same as a West Ruislip - High Wycombe ticket ?

Not always. Sometime the fares differ, and sometimes there is more than one appropriate boundary station. For example,
National Fares Manual NFM 06 CD said:
Boundary Zone 6 - Horsham route Any Permitted CDR £10.50 B1
Stoneleigh - Horsham route Dorking CDR £9.20 B1
Cheam - Horsham route Not Clapham Jn SDR £9.80
Coulsdon South - Horsham route Any Permitted CDR £10.00 B1
Oddly, there's a Cheam - Horsham route Not Clapham Jn CDR £9.90 B1 and a Coulsdon South - Horsham route Direct CDR £10.60 B1. I suspect neither of these sell very well.
i.e. the BZ6 - High Wycombe ticket costing the same as a West Ruislip - High Wycombe ticket ?
They have the same £6.50 SDS and £9.00 SDR, but the following fares exist only from the named origin:
National Fares Manual NFM 06 CD said:
West Ruislip - High Wycombe route Any Permitted WSP £7.20 (LONDON WASPSRFC)
Boundary Zone 6 - High Wycombe route Any Permitted SVR £12.60 4Y

Have the rules changed in recent years?

Here is the text from the latest fare manual available to the public (NFM98

"Day Travelcards can be used in combination with Boundary
Zone tickets (see section K.3) for journeys which extend
beyond the London Fare Zones boundary, whether or not the
train calls at the point where the ticket holder changes from
one ticket to the other. When used in combination with a
point-to-point ticket, the train must call at the station where
the ticket holder changes from one ticket to the other."

That text was missing from NFM97.

That suggests that the old rule of buying a ticket to the last station in Zone 6 is no longer permitted, unless the train stops there.

My understanding is that the current instruction to staff is:
Travelcard Excess Fares

Holders of any Day or longer period Travelcard may purchase single or return Excess Fares tickets for travel to a range of National Rail stations outside the London Fare Zones area. Tickets should be issued from the outermost Boundary Zone of the Travelcard held and must not be valid for travel after the date of expiry of the Travelcard held. Day Travelcard holders can only purchase a CDS, CDR, SDS or SDR Excess Fare.

Travelcard Excess Fares should only be issued in conjunction with a valid Travelcard for journeys to or from stations outside the zones covered by the ticket held. Passengers must keep Travelcards ready to present for inspection together with their excess fare ticket.

Excess fare tickets should be issued using the special ‘Boundary Zone’ locations, between the Boundary Zones and stations shown in your TIS or the Fares Finder. Passengers travelling to or from stations not shown in your TIS or the Fares Finder should be issued a point-to-point ticket for either
  • The last station in the outermost zone covered by their Travelcard (travel will then be subject to the National Conditions of Carriage, Clause 17 (combination of tickets)), or
  • London Terminals, the individual London station, or other named station as appropriate.
Excess Fare tickets must not be valid after the date of expiry of the Travelcard held. For example, holders of Day Travelcards can only purchase a CDS, CDR, SDS or SDR Excess Fare ticket valid for the same day as the Travelcard held.

Where possible excess fares, including those issued at a discount to the holders of valid Railcards, should be purchased before the journey starts, but they may also be obtained on the train or at the destination, subject to Penalty Fares Scheme regulations.

Care must be taken where more than one routeing exists for Excess Fares from Boundary Zones, and the correct ticket appropriate to the route and service required must be issued.

All Excess Fares tickets are subject to normal restrictions.
From 1997 to 2005 Clause 17 of the National Rail Conditions of Carriage covered combination of tickets. This is now in Clause 19.
