York has both linking the platforms.

Darlington has a subway leading in to town and a footbridge over the avoiding lines towards the car park.

Shipley has one of each serving different platforms - some changes require you to use both.

Keighley has a subway between the two KWVR platforms and a bridge connection to the main line platforms.

What others are people aware of? Please only count those that are available for passenger use.

Carlisle has a footbridge but the lifts go down into a subway.
As does Newcastle Central.

I wouldn't really count Reading, because AIUI the public subway and right of way is no longer within the station's area of responsibility.

New Cross and Orpington have both (in both cases the subway has steps only and the footbridge is fairly new to provide step-free access with lifts).

Hither Green has a footbridge across Platforms 1-4 (the South East Main Line), another footbridge across Platforms 4-6 (5 and 6 are on the Sidcup Line), and a subway with ramps which leads to exits on either side of the station, but I'm not sure if all platforms have access to the subway.

Clapham Junction has both to at least some platforms.

East Croydon has an overbridge with ramps (from the south end of the station where the ticket barriers are) and a more recent footbridge further north.

The old London Bridge had both on the high level (South Eastern) platforms but since the rebuild all access to those platforms is from the new concourse below.

Preston has already been mentioned, but it must be said that it has 2 subways in addition to the footbridge; one that's steps-only and a separate one for the lifts.

Portslade has both and a level crossing to boot.

Though the subway is outside the station so accessible by members of the public too.

Ascot. A subway links all three platforms and a bridge links platforms 1 & 2 (unless they extended the bridge lately)

Quite a few then.

Also Manchester Oxford Road - lifts connect via a subway.

Wasn't the subway at Newcastle just there for the use of Post Office staff any their trolleys of mail?
