"Signalling issue under investigation" is no different to "operational incident" other than it incorrectly confers some kind of negative connotation with the signalling system.
To my layperson ears, they are subtly different. Operational incident is hiding behind vagueness. Railway staff know it is really a euphemism for a SPAD or SPAR, but passengers probably won't. As I said, it's very close to the excuses London Midland use when they've screwed up.
I get why it is deliberately vague, so as not to implicate a driver or signaller in something, but it is deliberately vague.
And if a driver misinterprets a danger signal, or a signaller flips a green to red accidentally, that is an issue with the signalling system. People are part of the system.
As Chris M says, it comes across as "we have to tell you something, not that you deserve to know why you're delayed yet again, this is something, sod off and leave us alone".
In any case, no amount of moaning or bickering on here will change anything. Either get over it or lobby your mp/local toc.
Of course it won't change anything. As for complaining to the TOC...
