Thanks for letting us know the outcome. I am glad that you have been able to settle this out of court and they are willing to accept this payment and issue a final warning.

In terms of the TVM being out of order at the station boarding. Taking a picture is a good idea. Using the help point at the staiton is also a good idea as you can speak to a reaal person and make a note of their name. Also you don't need to do this but if you wanted, you could phone the customer relations of the TOC that runs the station and ask them for permission to travel. I've never done this personally but it is a reasonable phone call. Again, you could get the name of the person you spoke to and ask them to log your call. It might also speed them up in terms of fixing it if they see customers are calling in about the matter.

However, as soon as you get on the train, approach the guard (you will see what part of the train he/she is at when they operate the doors at the staiton) and say the TVM is out of order and you wish to buy a ticket.

My understanding is that when they go out of order it is logged so the staff on the gateline at Crewe would be able to check this.

Nobody needs to make any phone calls in order to obtain permission to travel from an untstaffed station.

The use of a help point (where available!) or a photo of a machine may make it easier for your to be clear on your case, but they aren't necessary and it's not a good idea to go too far out of your way to do these (primarily by risking missing the train). You merely need be as clear as you can that you were unable to pay at your origin and why.

They normally do - as long as the connection isn't broken.

I have taken a picture of the machine, whether that would help if I was stopped again I don't know.

It would only really help if the image includes evidence that it was taken just before you boarded the train (date on picture or other clear evidence), but in any case, the TVMs are monitored and the TOC will (should) know if one is out of service. On train staff are advised by the company when this arises

They are putting barriers up at Crewe now. I'm not sure what to do in future if I travel and the machine is out of order? Do I take a picture?.

The simplest thing is to be pro-active. I know it isn't always possible, but ATW operate Nantwich and when you get on the train the conductor/guard will be visible on the platform as it's his/her responsibility to check the doors are cleared before giving the driver the signal to proceed.

Try to board somewhere near the guard if possible and draw his/her attention to the fact that you need to speak with him/her.
