Unusual TfW services

Hi all, wondered if someone could inform me on what is going on with these diagrams?

I spotted this on Realtime Trains this morning and wasn't really sure what to think of it, especially as it said the unit had first and standard class, but currently only the Mk4s have that. There was no specific unit listed. Would someone be able to explain what is going on, as it seems very unusual to me.

Looks to me like a wrong button was pressed. 1W93 is terminating at Chester and think someone accidentally imputed Church Stretton instead, there is a continuation of the train with same headcode from Church Stretton: https://www.realtimetrains.co.uk/service/gb-nr:85800/2021-12-04/detailed

Not sure why shows as first and standard, unit on MN602 (Machynlleth depot) diagram so booked be a 158 but is covered by 150213 today.
