Unsafe practices

Can anyone still working within the rail industry explain to me why we carry on with the unsafe practices of blindly ignoring Standards and Procedures? It seems to have become endemic and I would like to believe it is as a result of commercial pressure but I have seen too many instances where it is detrimental to expediting projects to believe that is the case. Are people lazy or incompetent?

I would suggest you take this up with the revelant companies rather than asking a group of (non) professionals on here.
