Now that I've mastered the technology, here's a picture of Penrhyn Crossing on the Festiniog Railway as seen from an approaching train in 1960 :

The Welsh Highland Railway has a rail-on-rail 'flat crossing' over the local Network Rail standard gauge line. Does that count? !!!
