There's quite a few similar things where non-stop services are routed via routes they don't take - eg all the SE non-stop trains between Victoria and Bromley South are routed via the Catford loop on your map, when they typically go via Herne Hill.

@CW2 's point there in #57 is about beyond OOC. Extension to OOC = good as a lot of passengers will board there heading into London. Extension beyond = problems with crowding and needing more rolling stock.
It's two separate bridges, with a linking connection (long since permanently closed) - this photo shows it well ('landside' is the red bridge in the foreground, 'railside' is the white-and-red bridge behind, connection the stairs going over the roundel).

Presumably we only care about London services and are ignoring the branches. Amersham sees 2tph off-peak Chiltern, and 4tph peak (and the same with the Met line).
