It sounds as though it's likely to have been an issue with the specific card itself. Ideally I'd have wanted to try an alternative card, but if you don't have another card on your person then that's not much use!

I'm pretty sure that there isn't an online authorisation process that happens in that moment when a card is presented to a reader (validator). It's not fast enough. Rather, it gets sent to the backoffice and an authorisation happens then (10p in my experience), which might not be long afterwards...
